You are walking through your site and see this. There are five people working on this equipment and a shift change is about to happen. What do you do? What other things might you look for while you are here?

I cannot see the entire picture, but what I can see is the hatch in not on correctly! Not a proper Lockout!

Well the loto device is not correctly applied, there do not appear to be enough locks for all of the workers and I would bet their training records are appalling if any exist at all
Some good observations here, but to me, the primary issue is that the hasp is on there backwards. The switch could be thrown by removing only the lock that’s connecting the hasp to it.

The warning label on the LOTO device is missing. The hasp is not placed on the device properly. If there are 5 people working on the equipment, where are the other 4 locks? Also, the silver master lock is not OSHA-compliant because it’s the same as a regular master lock that many people use on their lockers, bags, etc. There are two locks, but one ID tag, so not sure what belongs to who.
As far as corrective actions, re-training on the proper site-specific and machine-specific LOTO procedures. Ensure there are standardized LOTO devices and that there are enough for everyone. If there are 5 people here, I’d suggest using group LOTO.
Ensure everyone is familiar with the process for shift change or change of personnel. Is there a transfer lock system in place, if needed?
Is this actually the only place that needs to be locked out? Or are there other energy sources?
I think it’s time for a periodic LOTO audit!

Haha "Lock out, clock out" - one of the comments on the reddit thread that made me chuckle. I mean, they have the locks over the disconnect, so at least there's that. I would say a quick LOTO refresher would fix the issue
