We actually just use a spreadsheet. It's old school, but it's easy enough to maintain, and obviously excel is super data-analytics friendly

There is a great product called Cority with which I have experience. It is expensive but you get dedicated support and you can build out lots of modules/features.

I am working with a system that is modular called BIS. Our main focus right now is in the onboarding and LMS features, but the system has options for safety management, electronic forms, driver info, documents, asset tracking etc.
Kind of like a box of Lego. you can build what you need. Costs depend on what features you want, but seemed reasonable compared to others we looked at.

We use Intelex which has many EHS apps in there platform including incident reporting. Not a fan. Outdated user experience and not that customizable
