The assessment is to be completed by observing the individual while they are assembling a flange as part of a job. The assessment is to be completed by the Maintenance/ Electrical Department Supervisor or their Foreman.

Flange tag installed prior to work begins
Flange tag is filled out with proper information
Proper Gasket is selected
Proper Bolts Selected
Flange surface inspected/cleaned
Alignment is within specification
Adhesive not used on gasket
Gasket properly seated
Bolts Lubricated
Proper bolting pattern followed
Incremental Tightening
Proper tightening tools used
Desired torque achieved
Assembly tag removed and sent to Bobby Joe

• All steps must be properly completed to pass the evaluation.
• Individuals that fail a portion should receive additional instruction and must be re-evaluated.
• Employees must be re-evaluated every 3 years.
