
Do you guys have a lock cutting procedure for LOTO?
Just curious, this topic was brought up today at the office and got me thinking. Do any of you allow cutting locks if someone loses a key, and if so what's the protocol?

We have a very precious procedure which includes calling employee, then manager, then department lead.

The following steps will be taken in the event that a lockout must be removed by other than the owner of the lock.
1. Identify the owner of lock: Equipment:

If owner of lock cannot be contacted, any maintenance supervisor may act as Emergency lock removal coordinator. Stop notification process after 1st contact is made.

Treat removal of lockout locks as you would an accident investigation. Ask: Who, What, Why & Where. Ensure that complete pre-start safety checks are completed by maintenance personnel. Do not assume that equipment is safe until all possible safety measures have been taken.
Note: The original copy of this form is to be forwarded to after completion. Notify Safety Manager via voice mail after such an event has occurred and advise the status. Please contact Safety Manager immediately with any questions that may arise or if assistance is needed.

Most sites we visit have a process if the lock needs to be removed and the individual assigned to the lock isn't on site. Typically there is a form with a 2 party sign off, stating that they attempted to and whether they did/did not make contact with the person. Also noting that they verified the space/area is empty and the person is not on site (gate badge logs help with this one).
