Common Scaffold modification missteps?
#1. Not clearing Scaffold before pulling tag.
I have been trapped more than once from Scaffold crew simply pulling a tag and starting modifications.
2. Mis label inspection tags from yellow to green?
Scaffold tie-in not to standards?

Thanks Norman - what industry are you in? We outsource all of our scaffolding construction, but it's something I would eventually like to internalize
I find a lot of mis-labeling, but most of it is labeling yellow instead of green because the contractor doesn't want to take responsibility for the user. They'll label it yellow and make it mandatory to tie off sometimes, which really slows things down.

I have seen contractors put bricks under the scaffolding feet...SO UNSTABLE, STOP! Correcting the behavior by talking to the people works. Tell them you want to see them go home tonight to their families and balancing on a brick is not worth their life. Usually works.

Here in Vegas, we see a TON of severely weathered and cracked planks. Also, connecting hardware takes a beating and components remain in use, but erectors just use wire ties instead of replacing the component.
I'm in a funky position, making these observations but without Competent Person authority. So my resolution step is usually to call the CP and ask him to meet me there, and then explain the hazard and ask him to abate it.
