Looks good, Fitz! Laying out the variety of questions they need to answer, on their end, will help them determine how you can better assist their organization. Well done!
Only thing I'd recommend is maybe creating a page on your website that has your picture and goes into your background. You have a link that goes to your LinkedIn page, but if they don't have a personal LinkedIn account, they may not be able to view your entire profile and see everything you want them to see (e.g., experience, credentials, etc.). By putting all of this on your website, it keeps them interacting with your website, and allows you to brag on your knowledge, experience, and whatever certifications you have - all in one spot.
Your posts you make on your website are a nice touch, as well! Keep up the good work!
