
What’s the best way to classify Incidents ?
I currently track hazard recognition , near miss , first aid , LTA, and recordable.
Then additionally track if they are spills, explosions , property damages, injuries , ect
An issue has arisen for me because how I currently track it I can have an explosion that’s a near miss (because no one got hurt)
However it’s an incident.
So when I pull (incidents vs near miss/hazard recognitions reported ) I get items like explosions counting as both.
What are some effectivetracking methods that are effective at isolating adverse events vs near miss?

I would never call an explosion a near miss, even if no one was hurt. Seems like these should all be incidents. It is definitely an unintended result of a work process.
I am actually more concerned that you have had 3 explosions in one year.

On the term "incident", that typically refers to preventability. OSHA elaborates on this on their website and basically the two terms are accident versus incident. Accident means there was no way to prevent it, whereas incident means it was preventable in some way. OSHA explains (and I agree that) most events are incidents because there's typically a way to prevent them from occurring in some capacity.
Careful, this could be considered HIPPA violation due to names!
