New safety tool

I’m working on a new safety tool and would like some professional feedback!
I am pairing the hazard wheel with the safety observation mentality. I have addd JSA like evaluation principals in order to ONLY target hazards, specifically looking for SIFS potential and if we have capacity for failure or not (if we fail , do we do so without injury )
If we dont have capacity , action items to correct the exposure must be taken by the direct manager.

What are your thoughts ?
