Unintentional Pervasiveness in Lockout

Some companies are quick to point at the employee and fix total blame on the injured employee that amputated their finger. They say, "We trained the employee to lockout and they did not!" These companies should remember that when they point a finger there is likely fingers pointed back at them. These organizations need to ask themselves, "How is our culture complicit in this accident? Why did this employee believe they had a choice not to follow lockout?" If employers are training their employees in lockout why do these serious accident occur?
