
The OSHA Cost of Not Following Good Lockout!
Just was reading this in the Region 5 Press Releases on OSHA.gov (Yeah I know I got to get out more!):

I do not know all the facts in this case, but with a little research I found that the company was cited on September 30, 2022 for 5 Serious, 1 Repeat, and 1 Willful all for Lockout. Looks like this visit was due to an amputation.

This food company had a another employee lose a leg and get another inspection (They are in the NEP for Amputations) a month ago. This time they get 12 Willful, 1 Repeat, 4 Serious, and 1 Other (that they are still fined $1348). All but the "Other" concern Lockout.

Looks like they have total fines of $2,174,673. They are contesting for sure the first fines of $245, 513.
Worse OSHA has put them in the Severe Violator Enforcement Program (SVEP) back in 2017. That sounds like NO FUN!

Looks like they had LOTO procedures but were generic, no employee initial or retraining for authorized employees, non-authorized employees performing LOTO, people removing other people's locks, and a handful of other items.
Sounds like the willful violations are deserving, and they need a serious management shakeup.
