I have been in manufacturing for 50 years! I have been a Supervisor - Plant Manager, and the Safety Guy to Human Resources/H&S Manager for all of my career. I have more LO horror stores than I care to remember. I have had to deal with many many amputations, but through the grace of God no fatalities! However, I missed one horrible experience by one week.
The week before I started work at a large iron foundry one of large shot blast cleaners was down to have the walls relined with Molybdenum Steel. These cleaners were as big as a two story house! The steel shot blasting the castings and walls of the shot room wears through the walls. Periodically the walls must be relined with a very dense hard substance (which is Molybdenum). We would have a external contractor to lay in the huge slabs of "Moly". There would be 20 of these contractors all over the machine, I remember seeing several hasps filled with locks (I would manage that area a couple years later). We would be all over the contractor as we only had 2 BIG cleaners and with one down, for sometimes 2 days it inhibited production a LOT!
My friend Steve was the boss over the area at this time. The contractor supervisor told Steve he could have the cleaner back after first break, 5:00 PM. Steve told his operator to check all the locks were off the machine at 5:15 PM, close the bulkhead, get the loading crew up on the machine and start the machine. Which he did.
At 5:45 PM the contractor supervisor approached Steve and said they were short one of their guys. On the way out of the plant the missing employee told a friend that he forgot his toolbox and went back to get it! They shut the machine down, locked it out, and went in. In the very back of the cabinet they found part of a hard hat, a little finger, and about half of the toolbox. 22-year old contractor.

I investigated a fatality at a facility you would recognize. A senior electrician LOTO a breaker on one wall. Turned 180 degrees and went to the switch gear that was lined up in side by side cabinets with other switch gear. His apprentice asked if he needed a zero energy check. He said “no, I know what I am doing”. Then he opened the wrong switch gear cabinet. His hand or a tool in his hand caused the arc flash. It was an ugly investigation.
