
Bad controls to implement (funny)
I was thinking it would be funny to discuss bad controls to implement for safety hazards!
I’ll go first - rigid wrist braces for everyone when they walk to and from their car in the winter so if they do fall, the rigid brace will prevent a wrist breaking lol your turn!

Here is an accident I had. see of you like my corrective action:
Accident: A supervisor was in the employee bathroom and doing his daily duty in a stall and on a commode. It was Third Shift about 3:00 AM, July. No one else in bathroom at the time. He supervises Mechanical Power Presses and Screw Machines (huge 6-8 spindle metal cutting machines). When suddenly he hears a page, "XXXXX (him) to Press 6204 STAT! " Knowing it is a Mechanical Power Press, and how dangerous they are, he jumps off the commode looking down to watch himself pull his pants up. Rams the top of his head into coat hook hanging on the stall door. Gashes his head and knocks himself out for a few seconds! Falls back on the to commode and wakes up with blood coming down into his face. Calls for help on his radio. Taken to the hospital where he has a concussion and 8 stiches on the top of his head. the accident occurred on Friday and he was back to work on sit down desk duty on Monday. No Lost Time but Recordable. Problem on machine was a scrap and rework problem.
Corrective Action: #1 Thought about making employees wearing hard hats in the bathrooms but settled for banning coat hooks on bathroom stall doors. all moved to the side walls of the stall. #2 Banned "Stat" calls on the PA unless absolute EMERGENCY. Supervisors all had radios that attached to the phone system, but they were knew at the time of accident.
