At the risk of sounding dumb because I don't actually work with this equipment, but to get the ball rolling, I would toss it. Looks like there's a crack?

I could not tell if it was the type that the gate opens for the loop and then you screw it shut or not. I was going to say if it was not I would not use it due to the chance the gate could open. I did do a little rock climbing belaying for a rec center when laid off years ago (I did very little climbing myself due to a fear of heights that I am not afraid to admit! :)) So was trying to remember everything as I do not work with a lot of fall protection. But I do see now the crack also that was mentioned.

Yeah That ones gonna get tossed with the crack on the locking gate, No Matter if you are following the NFPA, OSHA or ASTM.
I have separate bags for climbing, rappelling, and Hauling. Never ever do I mix my hardware.
Some of my tree climbers used to make me cringe when I watched them interchange their climbing gear and rope, that they pulled out of the same compartment that the gas, oil and chainsaws were stored in.
