
“Safety kiosk” for employee reporting
Hey all, hoping for some advice!
We use an internally built software system with the capability for any employee to report hazards, near misses, or other safety observations.
It’s not used often, since most operators don’t have computers at their work stations that can access the app. We used to have paper forms at work stations for employees to turn in, but keying those in is a nightmare.
I’d like to pitch “safety kiosks” to management. I imagine something like an iPad mounted on a wall or whatever, where employees can log in and report safety observations in their general work areas. I’d have a direct shortcut to the software available on there, and instructions posted nearby (as well as providing training & communication on how to use it obvs).
Does anyone hav anything similar to this process? Any pros/cons? Suggestions?

I think that's a great idea - plus it won't be very costly. I don't see how they can say no to a few iPads

I've seen this done before in facilities and it was great! Be sure that your teammates are protected from any operations while entering safety information. This can be achieved with bollards or other means of distance barricades. The last thing you want is for them to be distracted entering a concern with forklift traffic breezing by. I have also been at sites where the teammates had the option of putting the app on their own phone and this allowed them to attach pictures and enter the concern at their control centers (away from operations in the field).

What about an incident reporting app for smartphones?

We implemented a QR code for an easy form to fill out. Started it in July and have had great success with it. Everyone has a phone

I second the idea of QR codes! We use them all the time. I made them into posters and stickers and stuck them everywhere. They can just scan with their phones. We use Google classroom for surveys since it's free and easy. You could have the qr code take them right to the form

Why not go with an external software option that's mobile friendly? Everyone has a phone these days, might as well encourage data collection by making it as easy as possible?
Agree on the QR’s. We average 700 inspections, observations, and incidents per week. Each piece of mobile equipment has one, the area inspections each have one, and we have several codes throughout the plant for observations and incidents. Love it!
AI breaks down all the entries into pie charts. We dig in to the data bi-weekly to make sure we aren’t missing anything.
Hi Amanda- I have talked to several safety professionals who use this exact process with KPA EHS. It seems to work well having stations for employees to utitlize, and that way you can have everything stored in one place. It has made their program far more efficient.
