If the LOTO machines are attached to the actual machines/equipment, then the revision/review dates are listed at the top and new ones are printed off. If they are place in a binder together, I’ve used a cover page at the front of the binder to stare that they were all reviewed and the date that the final review was completed.

Update any changes, if any.

We have so many machines that new forms are not printed unless there was a change. The review date is rather documented in our database.

We have a semi-automated management system that assists us there. It's nice, would definitely recommend technology to help with the administrative side of LOTO

We have a sheet that we certify all procedures have been audited and any changes have been documented and training conducted.
