You walk up to your worksite and see this operation. The only PPE being worn on their face looks like a surgical mask.
What actions do you take? What concerns do you have? How would you leverage your safety management system in this situation?

Not sure that I would be concerned about what he’s wearing on his face. I would be more concerned about his use of the ladder to reach his work.
In looking closely he could probably do that work from the platform above.

I feel bad for this person! They should have just put up some scaffolding or gotten a big lift. Clearly they didn’t feel like dishing out the money

Improper use of the ladder is the first thing that sticks out. The Ladder should be folded out not leaned against, but in this situation a ladder creates a bigger fall hazard. The work should be done from a different location such as from the top of the surface.

What is he doing? I agree with everyone else on the ladder but is this more than 6 feet off the ground? I’d say some fall protection is in order. If the work could be done elsewhere I agree with Jeremy. But seriously what on earth is he doing? Doesn’t look like any work is being done.

Oh My!!! Improper use of a ladder. Fall Hazard concerns. No fall protection in place. I see this stuff all the time. Not that I should be seeing it. Some people just don't think or put way to much trust in things they shouldn't.

I would pull him down and give him training on proper PPE, Ladder use and Fall Protection Awareness. I would also ask him why he wouldn't feel scared in that kind of situation or why he would risk his life for a paycheck. Money is not worth dying.
