This is going to be a pretty hard thing to get an out of the box solution for, as all PSM is heavily process based, so it will have to be tailored to your specific site activities.

1910.119(h) is actually pretty generic, the key is evaluating the contractor performance, making sure they understand the hazards of the process, ensure all contract employees are trained on the job they are performing are made aware of the hazards and have access to process safety information to perform their jobs safely.

I actually just wrote my companies less than a week ago. Surprisingly easy, the OSHA citation listed in here from Austin is a great bases to start. I used the most common safety topics we expect our own employees to comply with and changed the wording. The rest was training requirements and authorization to disclose information for auditing purposes. That worked for our company dealing in Construction and satisfied our customers.

We have a process optimizer. He is in charge of leading our team in our 5S and Lean Six Sigma as well as so many other things.
His duties are really a site specific.
