
Most common violations you see with fall protection
What are some of the most common issues you guys see when it comes to fall protection (aside from people not wearing it)? Seen quite a few posts on fall protection lately so wanted to keep those discussions going.
One thing I see is people having a harness on, but not having it tied off anywhere. I also constantly see people climbing up the basket rails on lifts.

Good topic! These are what I see:
If employees run cable wires as fall protection acting as guardrails, they must be taught, most of the configurations I see they are loose and you can lift them up or down over 12 inches, they flex easily. This will not withstand 200 pounds of force and I would definitely not count on it to save my life. They have to be tightened regularly and not flex more than 3 inches in either direction (42+/-3').
Another one is floor openings for extension ladders (extension ladder going from inside a building, through the ceiling, to the roof). The ladder should have a coral built around it to prevent employees from walking straight into the hole.

Having a harness on and not wearing it properly. Leg straps down by their knees.
Using guardrails as anchor points. Not all guardrails are engineered to withstand the load required as an anchor point. Misunderstanding around qualifications to perform leading edge work vs typical working from heights. Lanyards being too long - not understanding the calculation requirements of a fall. Misunderstanding around safeguards needed when working around temporary holes in the floor (e.g. barricade, signs, hole watch, etc)
